Thank you so much for your participation in The Arc’s programs. To maintain quality services we rely on city grants and you can help us by providing the following:
Proof of Disability (written letter from doctor, social security award letters, supplemental security award letters, document signed by case manager, or the first page of and IEP from school)
Completed Public Service Participation Form for the city you reside in
These forms can be turned via email, fax or at our office at 1455 Fowler Street Richland, WA 99352.
